Out-of-Hospital Cardiac Arrest Locations ina Rural Community: Where Should We PlaceAEDs?

Abstract

Early defibrillation improves survival for patientssuffering cardiac arrest from ventricularfibrillation (VF) or ventricular tachycardia (VT).Automated external defibrillators (AEDs) should beplaced in locations in which there is a highincidence of out-of-hospital cardiac arrest (OOHCA).The study objective was to identify high-risk, rurallocations that might benefit from AED placement. Aretrospective review of OOHCA in a rural communityduring the past 5.5 years was conducted. The OOHCAsthat occurred in non-residential areas werecategorized based on location. Nine hundred, fortyOOHCAs occurred during the study period of which 265(28.2%) happened in non-residential areas. Of these,127 (47.9%) occurred in healthcare-relatedlocations, including 104 (39.2%) in extended carefacilities. No location used in this study had morethan two OOHCAs. Most (52.1%) non-residential OOHCAsoccurred as isolated events in 146 differentlocations. Almost half of the OOHCAs that occurredin non-residential areas took place inhealthcare-related facilities suggesting thatpatients at these locations may benefit from AEDplacement. First responders with AEDs are likely tohave the greatest impact in a rural community.
