The National Health Service instituted a “four0hour rule” which mandated that 98% of patients presenting to an emergency department (ED) must be seen, treated, and then admitted or discharged in under 4 hours. Acute medicine, a branch of general (internal) medicine, is a recent development in the United Kingdom. Acute medical units (AMUs) are an intermediate area between the ED and the downstream wards. Some AMUs have taken on the role of the ED-led observation units (OUs) while other larger hospitals and those where the AMU is not co-located within the ED often also have traditional ED-led OUs.
