A paradigm of surgical cytoreduction for women with grossly metastatic stage IV endometrial cancer analogous to that used in stage III ovarian cancer has been widely recommended in the absence of any prospective randomized data. While a number of retrospective series suggest that women with optimally cytoreduced endometrial cancer do better than those with suboptimally cytoreduced disease or those in whom cytoreduction surgery is not attempted, they are generally limited by the inclusion of heterogenous histologies with differing stages, and all are plagued by selection bias.
