This book has been a long time in gestation, and is the product of lengthy and at times demanding discussions and debates. Largely because he is based in Melbourne, Adam Sutton has assumed the task of editorial coordination and for liaison with the publisher. In the latter stages of the book, due to unforeseen circumstances, Rob White had to take responsibility for final rewrites and overall editing. While each of us is the first author of individual chapters (Cherney of 2, 4 and 5; White of 7 and 8; and Sutton of 1, 3, 6 and 9) arguments of this book reflect our shared views.
